HCP-Connect.com is a portal developed by HealthCare Partners for Employed and Contracted Clinicians to make their practice more efficient and improve the quality of care delivered to our patients. Through this portal a physician can access 3 separate applications:

  • Claims (Office Ally) - Submit Claims
  • Referrals (also known as HCP-Connect!) - Get Your Referrals Authorized
  • Physician Information Portal (PiP) - Get HCC and P4P Patients Lists, Detailed Patient Information including Referral, Diagnosis and Filled Rx Histories, Physician P4P Measure Scores & More!

HCP Connect is always evolving. As new functionality is developed and information is added, you will be notified by your regional Physician Liaison.
